Please complete all information below. Be sure to sign the waiver at the bottom of the form. Signature and prepayment are required
for table reservation. Applications should be received NO LATER THAN July
1st, 2005. Friday setup will be
available to dealers.
Special requests for table locations, (e.g., next to
another individual or group), should be noted in the “SPECIAL REQUEST” section
below and submitted as soon as possible.
Locations with electric outlets are limited. We will do our best to accommodate all table
holders and their requests, but we cannot guarantee any special table
location. Show may sell out quickly requests honored on a first come
first served basis.

additional table, up to a maximum of three helpers.
Helpers must
submit an NSA registration and pay appropriate fees to attend.

ALL TRADE FAIR EXHIBITORS MUST SIGN THE WAIVER BELOW:
I, the undersigned (Exhibitor), agree to waive all
claims against the National Stereoscopic Association (NSA) and/or its
individual members for any losses sustained by myself or my representative,
family, guest, or assistants. All items
are shown or offered for sale at the risk of the Exhibitor. Exhibitor agrees to authenticity of all items
sold. The NSA and/or its members assume
no liability to or for any person, thing or property.

received and processed, we will advise and confirm space assignment. When you check in for registration, a
schedule, and other information will be included in your registration package.
Check for table location with the trade fair chairman at setup. No refunds
after June 30th.
